ON THIS DATE, this contract is made between Sherry L. Allen- Blankenbeker (Seller), currently residing at Telephone number and THE BUYER (S) Currently residing at
Telephone number for the ownership of the following PET/COMPANION for the purchase price of

THE BUYER (S) agree (s) that this pet will not be used for breeding purposes, is sold for a pet or companion, and will be purchased with limited / un-limited AKC registration.

THE BUYER (S) agree (s) to pay any and all additional expenses, including shipping expenses, if required. Seller does not guarantee a purchased puppy's safe arrival if puppy is shipped out of state (the responsibility is through the shipper). The responsibility for safe arrival is not guaranteed by Seller for any shipping method other than Sellers personal hand delivery of the puppy to the Buyer. Seller will personally deliver this puppy at Buyer (s) expense. Traveling expenses must be paid prior to Seller delivering this puppy to Buyer (s). Travel expenses are non refundable should Buyer (s) refuse to accept this puppy upon Sellers delivery. THE BUYER (S) agree(s) to take possession of said puppy NO LATER then twelve weeks of age or this contract will become void and said puppy will become the sole property of Seller. Deposits are non-refundable.

THE BUYER (S) certifies that he/she is not acting as an agent for another individual in the purchase of this puppy and will not sell this animal, or any of its offspring, to any agent, pet store, puppy mill, kennel, or individual or agent of an individual, whose purpose is mass production, wholesaling or retailing.

THE BUYER (S) agree(s) to give first right of refusal to Sherry L. Allen-Blankenbeker should the Buyer (s) decide to sell this puppy. Any new owner must also agree to abide by the terms of this Contract.

HEALTH GUARANTEE: Sherry L. Allen-Blankenbeker guarantees this puppy against congenital defects until it reaches twelve (12) months of age. This includes the following conditions: epilepsy, deafness, blindness, hip displacia (debilitating), liver shunt, or heart murmur (grade 3 or greater). This twelve-month guarantee does NOT include minor illnesses such as worms, ear mites, hernias, hypoglycemia, allergies, yeast, and coccidiosis or giardia lamblia.

During this time, if this puppy should expire and the Buyer(s) wish to request a replacement puppy, Buyer(s) agree (s) to have an autopsy performed at Buyer's expense, by a licensed, practicing veterinarian, and to provide a certified copy of said autopsy to Seller via certified U.S. mail. Should the cause of expiration be confirmed through autopsy to be a congenital defect or to have occurred in the prenatal stage, upon the confirmation from Sellers personal veterinarian, seller agrees to apply the full purchase price toward the next available puppy.

This guarantee does NOT include loss due to veterinary services, self-given treatments, medicines or anesthesia. Any defects are to be detected BEFORE any treatments or services are given. This replacement guarantee will be terminated if the cause of death was determined to be abuse, ingesting a toxic substance, choking on a foreign object, food or any condition not confirmed by a veterinarian autopsy that did not occur from a defect in the prenatal stage of said puppy.

The Seller is not responsible for any veterinarian bills incurred. The Seller assumes no responsibility for this puppy after the delivery date for any medical expense, human allergies, training difficulties or behavior problems. Once the puppy is delivered, the buyer assumes all responsibility and cost incurred by this puppy.

THE BUYER (S) agree (S) that this puppy will reside at the home of the Buyer (s), have access to a fenced yard, if applicable, will not be chained or tethered, and will be provided with proper housing, a good quality diet, will keep all immunizations current according to AVMA guidelines, and properly administer heartworm preventative.

THE BUYER (S) hereby releases Sherry L. Allen-Blankenbeker, or any of her partners, business associates or relatives, from any and all liabilities and/or damages by fault of this puppy after this date of sale. These damages and liabilities include, but are not limited to, destruction of property and physical damage to any person or group of people.

If this contract is violated by THE BUYER (S), THE BUYER (S) hereby agree that possession of this puppy will be willingly released to Sherry L. Allen-Blankenbeker upon her demand, and that she is released from any obligations under this CONTRACT, and THE BUYER (S) hereby agree (s) that Sherry L. Allen-Blankenbeker has the additional right to receive punitive damages equaling but not exceeding the value of the sales price of this puppy, said price to be established by Sherry L. Allen-Blankenbeker and agreed to by both THE BUYER (S) and Sherry L. Allen-Blankenbeker at the signing of this CONTRACT. Punitive damages for violation of this contract shall be set at a value of $ , U.S. Dollars.

Should it become necessary to enforce any part of this contract by legal means, appropriate attorney's fees, court costs, and all travel expenses will be the responsibility of THE BUYER (S). This CONTRACT will be deemed a legally binding Contract, and under the jurisdiction of the state of Georgia, in the county of Rockdale.

I/WE, THE BUYER (S), hereby certify that I/we have read this Contract in full, understand it's complete contents, and agree to it fully.
